What do I need from you to claim on my insurance?

Most health insurance providers require a signed and stamped insurance claim form. Your clinician will complete this for you during your consultation. Your provider will also need the receipt to show proof of payment for your session, which our reception will provide once you have paid at the start or end of your session. Should your provider require more information, please let reception or the clinician know and we will endeavor to provide what we can in order for you to have a smooth reimbursement process.
